WHY THIS MATTERS

THE PURPOSE OF THIS PROGRAM IS TO:
- Have your blood pressure and BP log checked by a medical professional
- Detect high blood pressure early
- Monitor existing hypertension
- Avoid unnecessary travel to the medical office
- Stay connected to trusted healthcare providers
- All at no cost to you
HOW IT WORKS:
- Have your first visit to one of the Sheep Inc Health Care Center locations, if you haven't already.
- Find a participating pharmacy near you. (We are working on adding more!)
- When medication refills are needed, have your blood pressure and BR log checked.
- Receive simple, easy-to-understand educational materials and information about heart
health from a medical professional. - If your readings are elevated, Sheep Inc. will follow up and coordinate any needed care.
